直肠癌肝转移伴原发性肝细胞癌1例

摘要(Abstract):
直肠和肝脏部位的双原发癌为少见病例,临床上在未进行手术切除后的病理学检查前,且考虑到肝转移是直肠癌最常见的转移瘤,若仅仅结合患者的病史,依靠影像技术和实验室检查极易将肝脏部位后来发生的原发性肝癌误诊为直肠癌术后肿瘤转移,从而增加了误诊的可能性。准确鉴别转移性肝肿瘤与原发性肝肿瘤具有重要临床意义,因误诊可能导致对转移瘤采取不恰当的系统性治疗方案,而原发性肝细胞癌患者则可能通过根治性切除术获得治愈机会,从而显著影响患者预后。所以对于2种或者2种以上的原发性恶性肿瘤的相互鉴别及明确诊断,病理组织学和免疫组织化学至关重要。本文回顾性报告了我院的1例76岁男性患者,在进行直肠癌根治术7个多月后发生肝转移且并发原发性肝细胞癌的病例,并对其临床病理特征进行分析,为临床诊断类似病例提供相关诊治经验。
